                    <title>Background</title>
                    <p>Early initiation of antenatal care (ANC) during the first trimester is crucial for timely identification and management of pregnancy-related complications, provision of preventive interventions, and improvement of maternal and neonatal outcomes. The World Health Organization recommends that pregnant women should initiate ANC within the first 12&#x00a0;weeks of gestation. Despite global and national efforts, Ethiopia remains below the WHO recommendations for timely initiation and adequate utilization of antenatal care services due to barriers in access, awareness, and service quality. Understanding these factors is essential for designing targeted interventions that enhance continuity of care. Therefore, this study aimed to identify determinants of Early ANC initiation among pregnant women attending Bule Hora University Teaching Hospital, West Guji Zone, Oromia, Ethiopia.</p>
                </sec>
                <sec>
                    <title>Methods</title>
                    <p>A facility-based unmatched case&#x2013;control study design was conducted from June to July 2025, among 456 pregnant women attending ANC services, including 114 early ANC initiators (cases) and 342 late initiators (controls). Systematic random sampling was used to choose the participants. Data was collected via structured and a pre-tested questionnaire using the KoboCollect app. The data were cleaned and coded before being entered and analyzed with SPSS v27. Binary and multivariable logistic regression analyses were conducted to assess the determinants of early ANC initiation. Adjusted odds ratios (AORs) with 95% confidence intervals (CIs) were calculated to measure the strength of associations. Statistical significance was declared at p&#x00a0;&lt;&#x00a0;0.05.</p>
                </sec>
                <sec>
                    <title>Results</title>
                    <p>Of the 448 respondents 112 were cases, while 336 were controls with 98.2% response rate. Early ANC initiation was associated with younger age 15&#x2013;24&#x00a0;years (AOR&#x00a0;=&#x00a0;2.79, 95% CI: 1.12&#x2013;6.96), good ANC knowledge (AOR&#x00a0;=&#x00a0;4.19, 95% CI: 2.24&#x2013;7.82), waiting times (AOR&#x00a0;=&#x00a0;4.02, 95% CI: 2.23&#x2013;7.24), polygamous marriage (AOR&#x00a0;=&#x00a0;3.74, 95% CI: 1.53&#x2013;9.16), and community support (AOR&#x00a0;=&#x00a0;3.63, 95% CI: 1.44&#x2013;9.17).</p>

                <sec>
                    <title>Conclusion and recommendations</title>
                    <p>Early ANCs were significantly associated with maternal age, ANC knowledge, waiting time, marital structure, and social support. Promoting timely ANC initiation requires improving community engagement, improving service efficiency, and bolstering education. Therefore, health extension programs on early ANC initiation should be strengthened by giving priority to community-based interventions and male partner involvement to improve maternal health service utilization.</p>

In the Abstract and Introduction, the conclusion was re-framed to correctly identify positive drivers of early care rather than implies negative factors promoted it, specifically highlighting maternal age, ANC knowledge, shorter waiting times, marital structure, and community support as key determinants. The introduction was also updated to replace a generic gap statement with site-specific context focused on rural pastoralist and agro-pastoral settings in Southern Oromia, alongside a minor grammatical correction stating that "Ethiopia remains below the WHO recommendations." In the Methodology, text was added to explain that data were captured electronically via KoboCollect on Android tablets, validated in real time with built-in skip patterns and range restrictions, and synced daily to a secure KoboToolbox server before being exported to SPSS version 27. To address data reliability concerns given that 20% of the sample had no formal education, the text now clarifies that face-to-face, interviewer-administered questionnaires were conducted in the local language (Afaan Oromo) by trained health professionals, utilizing witnessed verbal consent for illiterate participants and written consent for literate participants to minimize response bias. Finally, in the Results section, a critical error was corrected by reversing the textual interpretation of the Adjusted Odds Ratios to properly align with the data coding.</p>

            <title>Introduction</title>
            <p>Antenatal care (ANC) is a healthcare service that provides essential interventions to decrease pregnancy related problems, provide birth, emergency preparedness counseling, and improve new born health outcomes and neonatal morbidity and mortality.
                <xref ref-type="bibr" rid="ref1">
                    <sup>1</sup>
                </xref> The world health organization (WHO) defines ANC as the care provided by skilled health care professionals to pregnant women and adolescent girls to ensure the best health conditions for both mother and fetus during pregnancy.
                <xref ref-type="bibr" rid="ref2">
                    <sup>2</sup>
                </xref> ANC provides opportunities for early detection and management of pregnancy-related complications, nutritional counseling, birth preparedness, disease prevention, and health promotion interventions. Early initiation of ANC enables healthcare providers to identify obstetric risks at the earliest stage of pregnancy and initiate appropriate interventions before complications become severe. ANC originated in the early 20
                <sup>th</sup> century as a preventive strategy to reduce maternal and perinatal mortality, evolving over time into a comprehensive approach that includes health education, screening, and management of pregnancy-related risks.

            <p>Globally, pregnancy-related maternal and neonatal mortality remain major public health challenges, predominantly in low- and middle-income countries.

                </xref> Despite substantial progress, recent data highlights that these issues remain alarmingly prevalent.

                </xref> According to the WHO, approximately 287,000 women died from pregnancy-related causes in 2021, while nearly 2.5 million neonatal deaths and 2.4 million stillbirths were reported during the same period. Sub-Saharan Africa accounts for the largest proportion of these deaths. Ethiopia continues to experience substantial maternal and neonatal health burdens despite improvements in healthcare access and expansion of maternal health services.

            </p>
            <p>The WHO currently recommends that pregnant women initiate ANC within the first 12&#x00a0;weeks of gestation and receive at least eight contacts during pregnancy to enhance maternal and fetal outcomes. However, timely ANC initiation remains suboptimal in Ethiopia. Evidence from national surveys indicates that a significant proportion of pregnant women initiate ANC during the second or third trimester. Delayed ANC initiation decreases opportunities for early screening of maternal complications, nutritional interventions, vaccination, counseling, prevention of mother-to-child transmission and early detection of pregnancy-related risks, and counseling on birth preparedness and complication readiness. Socio-economic status and education levels are significant predictors of ANC utilization. Lower socio-economic status often correlates with reduced access to healthcare services, including ANC, due to financial constraints and limited healthcare infrastructure.

            <p>Additionally, educational attainment influences health literacy, affecting women&#x2019;s ability to seek and adhere to ANC recommendations.

                </xref> Cultural practices and accessibility issues also play a crucial role in determining ANC uptake. In many communities, traditional beliefs and practices may either discourage or delay seeking formal medical care, including ANC.

                </xref> Furthermore, geographic and logistical barriers, such as the distance to healthcare facilities and inadequate transportation, exacerbate the challenge of accessing timely and regular ANC services.

            </p>
            <p>Previous evidence from studies conducted in Ethiopia and other African countries have identified multiple determinants of Early ANC initiation, including maternal age, educational status, socioeconomic conditions, and knowledge of ANC services, parity, pregnancy purpose, cultural beliefs, community support and engagement, accessibility of healthcare facilities, and quality of maternal healthcare services. Health system-related barriers such as long waiting times, poor provider-client interaction, distance and inadequate service organization also contribute to delayed ANC initiation.</p>
            <p>Even though several studies have examined early ANC utilization in Ethiopia, findings differ across geographic and socio-cultural contexts. West Guji Zone has unique socio-cultural and demographic characteristics, including agro-pastoralist communities, variations in healthcare accessibility, and differing community norms related to maternal healthcare utilization. Furthermore, limited evidence exists regarding determinants of Early ANC initiation in the study area. Understanding these determinants is essential for designing context-specific interventions aimed at improving maternal healthcare service utilization and reducing adverse maternal and neonatal outcomes.</p>
            <p>Therefore, this study aimed to identify determinants of early antenatal care initiation among pregnant mothers attending ANC services at Bule Hora University Teaching Hospital, West Guji Zone, Oromia, Ethiopia. The findings will equip healthcare providers and facilities to design effective service delivery strategies to increase early ANC service utilization, help to identify institutional barriers linked to maternal and newborn mortality, and support policymakers, planners, and program managers in enhancing ANC services. Moreover, this research will serve as a foundation for future studies focused on improving early ANC initiation and optimal ANC continuity of care in the region.</p>
        </sec>
        <sec id="sec6">
            <title>Methods and material</title>
            <sec id="sec7">
                <title>Study setting and design</title>
                <p>This study was carried out in Bule Hora Town, which is located West Guji Zone of the Oromia region in the lowlands of southern Ethiopia. According to the West Guji Zone health and demographic surveillance department dataset, this is derived from the 2007 census by 2.4% inflation.
                    <xref ref-type="bibr" rid="ref14">
                        <sup>14</sup>
                    </xref> The town, which is located 465 kilometers from the capital city of Addis Ababa along the main connecting road, is under zonal administration and has a total population of 157,148. The town is characterized by an urban setting, a mostly agrarian economy, and a population that is mostly from the Oromo ethnic group, namely the Guji agro-pastoral community. A facility-based unmatched case-control study design was employed. To address the determinants of early antenatal care initiation, the study followed specific procedures conducted between June and July 2025.</p>
            </sec>
            <sec id="sec8">
                <title>Study participants and sampling procedure</title>
                <p>The study population consisted of pregnant women who attended ANC at Bule Hora University Teaching Hospital during the study period and were randomly selected. Cases were defined as women who initiated ANC before 12&#x00a0;weeks of pregnancy (early initiators), while controls were those who started ANC at or after 12&#x00a0;weeks (late initiators).

                    </xref> Eligible participants were women aged 18 and older, as well as mature minors under 18, who had resided in the study area for more than six months. Women with unknown gestational ages at their first visit, hearing or speech impairments, serious pregnancy complications, or mental illness were excluded from the study. The study participants were approached by the principal investigator and interviewers with extensive experience in quantitative research.</p>
            </sec>
            <sec id="sec9">
                <title>Study variables</title>
                <p>Dependent/Outcome variable: Early ANC initiation, defined as initiation before 12&#x00a0;weeks of pregnancy (early initiators/cases&#x00a0;=&#x00a0;1), versus initiation at or after 12&#x00a0;weeks (late initiators/controls&#x00a0;=&#x00a0;0).</p>
                <p>Independent/Exposure variables: Sociodemographic factors (age, residence, marital status, personal and husband&#x2019;s education and occupation, income, and distance to a health facility); knowledge-related factors (awareness of ANC importance, timing, number of visits, services, danger signs, and information sources); individual and obstetric factors (parity, gravidity, previous and current ANC timing, delivery location, history of abortion/stillbirth, past ANC use, family planning, pregnancy planning, complications, and transportation access); and finally, health facility and cultural factors (facility proximity, service costs, drug availability, waiting times, provider attitude, husband accompaniment, community support and norms, and cultural barriers).</p>
            </sec>
            <sec id="sec10">
                <title>Sample size and sampling procedures</title>
                <p>The sample size was calculated using a double population proportion formula in Epi-Info version 7.2.2.6, based on a key determinant from prior literature which is educational level (Exposure proportion: 
                    <bold>35.7%</bold> among controls (late ANC initiators after 12&#x00a0;weeks).

                        <sup>16</sup>
                    </xref> The first calculation produced 416 participants with a power of 90%, a 95% confidence level, a case-to-control ratio of 1:3, an exposure proportion of 35.7% among controls, and an adjusted odds ratio of 2.15. The final sample size was 456 (114 cases and 342 controls) after accounting for a 10% non-response contingency. Simple random sampling was used to choose study participants from the hospital&#x2019;s antenatal care register using computer-generated random numbers generator to select eligible pregnant women.</p>
            </sec>
            <sec id="sec11">
                <title>Data quality control</title>
                <p>Questionnaires were refined before deployment on KoboToolbox. After deployment on digital KoboToolbox. The questionnaire was pretested among 23 (5%) participants at a neighboring health facility before the actual data collection to ensure clarity, consistency, and applicability of the tool to ensure tool use, data accuracy, and ethical procedures for two days. Internal consistency of the knowledge assessment items was evaluated using Cronbach&#x2019;s alpha coefficient, and the reliability value was found to be acceptable (Cronbach&#x2019;s alpha&#x00a0;=&#x00a0;0.78). Built-in validation checks (such as skip logic, range limits, and required fields) were applied to minimize entry errors. Supervisors reviewed submitted forms daily for completeness and consistency, whereas the principal investigator performs random cross-checks and downloaded the data periodically to identify anomalies or missing values. After collection, all data was cleaned, verified for completeness, and exported to statistical software for final consistency checks before analysis.</p>
            </sec>
            <sec id="sec12">
                <title>Data collection procedures</title>
                <p>The data was collected using face-to-face interviews from pregnant women who can provide informed consent using Kobocollect app. The questionnaire was adapted from Ethiopian Demographic and Health Survey and different related literature.

                    </xref> Informed consent was obtained before conducting the interviews. The data collectors are professionally Nurses or health officer, or midwives have experience of working in the ANC unit from other facilities were participated and one supervisor was recruited from zonal health office.</p>
            </sec>
            <sec id="sec13">
                <title>Data analysis</title>
                <p>All complete questionnaires were checked for accuracy, consistency, and completeness before being entered into KoboToolbox and exported to SPSS version 27 for analysis. To find factors linked to Early ANC initiation, descriptive analysis like percentage, frequency and mean was computed to show the data characteristics.</p>
                <p>Binary and multivariable logistic regression analysis was used to identify associations between dependent and independent variables. Variables associated with early ANC initiation at p&#x00a0;&lt;&#x00a0;0.25 in bivariable analysis, together with variables supported by previous literature and biological plausibility, were considered for multivariable logistic regression analysis. To minimize model overfitting, the number of predictors included in the final model was evaluated considering the events-per-variable recommendation for logistic regression. Finaly age, Knowledge, waiting time, Marital structure, Community support, Residence, Education was considered for multivariable regression. The variables that show p value &lt;0.05 were declared as statistically significant variables in multivariable logistic regression analysis.</p>
                <p>The degree of association between independent and dependent variables was assessed using crude OR and adjusted OR (AOR) with 95% CI. The Hosmer Lemeshow test was used to check the model fitness, the Hosmer&#x2013;Lemeshow goodness-of-fit test indicated adequate model fit (p&#x00a0;=&#x00a0;0.80). Multicollinearity was checked (variance inflation factor&#x00a0;&lt;&#x00a0;10), indicating that there was no multicollinearity among the variables in this study.</p>
            </sec>
            <sec id="sec14">
                <title>Operational definitions</title>
                <p>

                    <bold>Antenatal care:</bold> ANC is defined as the care provided by skilled health care professionals to pregnant women and adolescent girls to ensure the best health conditions for both mother and baby during pregnancy.

                <p>Early ANC booking was defined as those pregnant women initiation ANC before or at 12&#x00a0;weeks of pregnancy, whereas, late ANC initiation was defined as those pregnant women starting ANC after 12&#x00a0;weeks of their pregnancy.                 <p>

                    <bold>Cases:</bold> Cases were pregnant women visited the ANC unit before 12&#x00a0;weeks of gestational age (ie, early initiators of ANC) while, controls were pregnant women who initiate ANC after 12&#x00a0;weeks.</p>
                <p>

                    <bold>Knowledge of ANC</bold>: Was assessed using structured questions covering early booking, danger signs, pregnancy complications, timing and importance of ANC, ANC services, and gestational age. Participants scoring &#x2265;50% of the total knowledge questions were categorized as having good knowledge, while those scoring &lt;50% were categorized as having poor knowledge.

                </p>
                <p>

                    <bold>Long Waiting Time</bold>: Waiting time was defined as the cumulative time a pregnant woman spent waiting at registration, triage, laboratory/diagnostic units, and before consultation with the healthcare provider during the ANC visit. Based on previous Ethiopian outpatient service studies, waiting time was categorized as short (&lt;120&#x00a0;minutes) and long (&#x2265;120&#x00a0;minutes).

            </sec>
            <sec id="sec15">
                <title>Ethical consideration</title>
                <p>Ethical clearance was obtained from Bule Hora University following the Declaration of Helsinki and was approved by Institutional Research Ethics Review Committee (BHU/IRERC) with protocol number of BHU/IRERC/030/17. A support letter was obtained from the Bule Hora Town health department to the hospital. All participants were informed about the purpose and the procedures of the study. All responses were kept confidential and anonymous. The study protocol was approved by the Bule Hora University Institutional Review Board (IRB). Prior to enrollment, informed consent was obtained from all participants: written consent was obtained from those with formal literacy, while witnessed verbal consent was obtained from participants unable to read or write.</p>
            </sec>
        </sec>
        <sec id="sec16" sec-type="results">
            <title>Results</title>
            <sec id="sec17">
                <title>Socio-demographic characteristics</title>
                <p>Four hundred forty-eight pregnant women participated in the study (112 cases and 336 controls) with a 98.4% response rate. The mean age of the respondents was 27.42&#x00a0;years (&#x00b1;5.72 SD). Most participants were urban residents, 394 (87.9%). The majority were married, 441 (98.4%), with slightly fewer in controls, 108 (96.4%) vs. 333 (99.1%) in cases. Secondary education (9&#x2013;12 grades) was the most common level, 123 (27.5%), and housewife was the dominant occupation, particularly among controls, 208 (61.9%) vs. 37 (33.0%) in cases. Husbands of 142 (31.7%) participants had higher education, slightly more among controls, 41 (36.6%). Less than 10&#x00a0;km distance to nearest health facility was reported by 321 (71.7%) participants, more in cases, 108 (96.4%) vs 213 (63.4%) in controls (

                <title>Knowledge related characteristics of the participants</title>
                <p>Most participants knew the importance of early ANC, 436 (97.3%), and its benefits, 433 (96.7%). Knowledge of Early ANC initiation was higher among 106 cases (94.6%), than controls, 154 (45.8%). Very small number of controls, 67 (19.9%), knew the correct time to start ANC (&lt;12&#x00a0;weeks) or first trimester, compared to 71 (63.4%) cases. Regarding the benefits of early ANC initiation, monitoring maternal and fetal health was reported by 106 (94.6%) cases, and by 154(45.5%) controls. Regarding the benefits of early ultrasound imaging before 24&#x00a0;weeks was reported by 111 (99.1%), while 233 (73.3%) significantly reported high by cases. Regarding the recommended number of ANC visits, most participants believed that less than four visits were sufficient, reported by 82 (73.2%) cases and 194 (57.7%) controls. In contrast, awareness of the currently recommended 8 or more ANC visits was very low, reported by only 2 (1.8%) cases and 3 (0.9%) controls. Knowledge of pregnancy danger signs was 189 (42.2%), with vaginal bleeding being the most recognized sign, 125 (66.1%). The main sources of ANC information were community health workers, 170 (37.9%), and health workers, 116 (25.9%) (
                    <xref ref-type="table" rid="T2">
Table 2</xref>).</p>

                <p>Knowledge of ANC was assessed using 12 structured questions covering timing of ANC initiation, recommended number of visits, ANC services, and pregnancy danger signs. Each correct response was given a score of 1 and incorrect responses 0. The total score was converted into a percentage, and respondents scoring &#x2265;50% were classified as having good knowledge, while those scoring &lt;50% were classified as having poor knowledge.</p>
                <p>As shown in the figure below, overall knowledge of ANC was higher among early ANC utilizers (cases) with 92 (82.1%) demonstrating good knowledge, compared to 164 (48.8%) of late utilizers (controls). Overall, 256 (57.1%) of participants had good knowledge, while 192 (42.9%) had poor knowledge of ANC (
                    <xref ref-type="fig" rid="f1">
Figure 1</xref>).</p>

                <title>Obstetric and individual related factors</title>
                <p>Out of total participants, 42 (37.5%) mothers with cases and 97 (28.9%) mothers with controls were primigravida and 309 (69.0%) multigravidas. One third of case 31(27.87) and 104 (31.0%), respectively were given birth. In their last pregnancy, only 49 (17.2%) initiated ANC in the first trimester, while 142 (49.8%) started in the second trimester, and 29 (10.2%) in the third. Place of previous delivery showed that 12 (10.0%) of the mother with cases and 57 (17.0%) of mothers with controls had a history of birth at home for the preceding birth, whereas 245 (54.7%) gave birth at a health institution and 9 (2.0%) on the way. A total of 70 (15.6%) had a history of abortion and 33 (7.4%) experienced stillbirths. Family planning use before pregnancy was reported by 250(55.8%) and ANC attendance in a previous pregnancy by cases and 201 (63.8%) controls. Pregnancy was planned by 331 (73.9%).</p>
            </sec>
            <sec id="sec20">
                <title>Health facility and cultural related factors</title>
                <p>Among the participants, 309 (69.0%) reported proximity to health facility, with a higher proportion among cases 101 (90.2%) compared to controls 208 (61.9%). Waiting times (&gt;120&#x00a0;minutes) were experienced predominantly among controls 169 (50.3%) versus only 20 (17.9%) cases. Most participants rated provider attitudes as good or very good: 421 (94.0%). In terms of socio-cultural and support factors, husband accompaniment to ANC was reported by 221 (52.1%), and 321 (71.7%) were exposed to maternal health messages through media. Perceived community support for ANC was reported by 106 (94.6%) cases and 253 (75.3%) controls, and 397 (88.6%) believed ANC attendance was a community norm. Decision-making regarding ANC was self- or jointly made by 403 (90.0%) participants.</p>
                <p>Perceived community support for ANC was measured using a Likert-scale questionnaire, and total scores were summed. Participants were classified as having &#x201c;good&#x201d; or &#x201c;poor&#x201d; support based on whether their score was above or below the mean.</p>
            </sec>
            <sec id="sec21">
                <title>The determinants of Early ANC initiation in logistic regression analysis</title>
                <p>In the bivariate analysis, variables associated with early ANC and (p&#x00a0;&lt;&#x00a0;0.25) were considered for the multivariable logistic regression. After adjusting for potential confounders, the multivariate logistic regression analysis showed that maternal age, knowledge of ANC, waiting time before consultation, marital structure, and community support were significantly associated with Early ANC initiation. Our study showed that women aged 15&#x2013;24&#x00a0;years were nearly three times more likely to initiate ANC early compared with women aged 35&#x2013;44&#x00a0;years (AOR&#x00a0;=&#x00a0;2.79; 95% CI: 1.12&#x2013;6.96). The odds of early initiation of ANC visit among pregnant women with good knowledge of ANC (AOR&#x00a0;=&#x00a0;4.19; 95% CI: 2.24&#x2013;7.82) were four times more likely to initiate early ANC (&lt;12&#x00a0;weeks) compared with women who had poor knowledge. Women who waited less than 120&#x00a0;minutes before provider consultation were more likely to initiate ANC early compared with women who experienced waiting times of 120&#x00a0;minutes or more (AOR&#x00a0;=&#x00a0;4.02; 95% CI: 2.23&#x2013;7.24). Women in monogamous marriages had nearly 4 times higher odds of initiating ANC early compared with women in polygamous marriages (AOR&#x00a0;=&#x00a0;3.74; 95% CI: 1.53&#x2013;9.16). Similarly, women who reported good community support for ANC were more likely to initiate ANC early compared with women who reported poor community support (AOR&#x00a0;=&#x00a0;3.63; 95% CI: 1.44&#x2013;9.17) (

            <title>Discussions</title>
            <p>This study investigated several determinants of early antenatal care (ANC) initiation among pregnant women attending in the Bule hora university teaching hospital, including maternal age, knowledge of ANC, marital structure, waiting time before seeing a provider, and perceived community support. After adjustment for confounders, these factors remained significantly associated with early ANC initiation, highlighting the combined influence of individual, social, and health system-related determinants. The study results revealed that younger women (15&#x2013;24&#x00a0;years) were more likely to initiate ANC early compared with older women. This study findings are in consistent with a study conducted in Central Ethiopia,
                <xref ref-type="bibr" rid="ref21">
                    <sup>21</sup>
                </xref> in Southwest Ethiopia
                <xref ref-type="bibr" rid="ref22">
                    <sup>22</sup>
                </xref> and in Shisong, Cameroon.
                <xref ref-type="bibr" rid="ref23">
                    <sup>23</sup>
                </xref>
            </p>
            <p>Younger women are more exposed to maternal health information through educational institutions, media, and digital communication platforms. In contrast, older women perceive themselves as experienced due to previous pregnancies experiences and therefore underestimate the importance of early ANC initiation. This finding suggests that maternal health education should target women across all reproductive age groups, particularly multiparous and older mothers who delay care because of perceived pregnancy familiarity.</p>
            <p>Maternal knowledge of ANC was a strong independent predictor of early ANC initiation. Good knowledge of ANC was strongly associated with Early ANC initiation. Women who recognized the importance of timely ANC initiation, pregnancy related danger signs, and recommended ANC follow-up schedules were more likely to seek care early. This finding inconsistent with studies conducted in Central Ethiopia,
                <xref ref-type="bibr" rid="ref21">
                    <sup>21</sup>
                </xref> in Addis Ababa city, Ethiopia,
                <xref ref-type="bibr" rid="ref24">
                    <sup>24</sup>
                </xref> and in Northwest Ethiopia.
                <xref ref-type="bibr" rid="ref25">
                    <sup>25</sup>
                </xref> This stresses the critical role of health literacy in shaping maternal health-seeking behavior and greater knowledge may contribute to influence care-seeking behavior to recognize pregnancy-related risks and increases perceived benefits of preventive healthcare measures. However, it is also important to recognize the possibility of reverse information flow in facility-based studies, where women initiating ANC early subsequently receive more counseling and health education. Therefore, strengthening community-based health education and reinforcing counseling during health facility visits is essential to improve Early ANC initiation.</p>
            <p>Regarding health system factors, the analysis indicated that shorter waiting time before provider consultation was positively associated with early ANC initiation. Women who perceived ANC services as efficient and accessible may be more motivated to seek care during early pregnancy. This study result was in line with a study conducted in Addis Ababa, Ethiopia,
                <xref ref-type="bibr" rid="ref26">
                    <sup>26</sup>
                </xref> southwest Ethiopia,
                <xref ref-type="bibr" rid="ref27">
                    <sup>27</sup>
                </xref> and Cameroon.
                <xref ref-type="bibr" rid="ref23">
                    <sup>23</sup>
                </xref> Long waiting times might discourage pregnant women&#x2019;s early health-seeking behavior, as women may perceive health facilities as inefficient or inconvenient. This perceived inefficiency, transportation challenges, competing domestic responsibilities, and opportunity costs might affect the service utilization grossly. This suggests that improving service efficiency, appointment systems, and patient flow management could enhance Early ANC initiation and overall maternal satisfaction.</p>
            <p>The study also revealed that women in monogamy marriages had higher odds of early ANC initiation compared with those in polygamy marriages. This study result was supported with a study previously conducted in Ugandan women.

                    <sup>28</sup>
                </xref> The significant association between lower use of early ANC and polygamous marriage may be due to women being second or third wives in the household, which can lead to fear of accessing healthcare services or prioritizing the needs and business of their partners over their own health. This finding highlights the importance targeted maternal health education and empowerment interventions may help improve early ANC initiation among women in polygamous unions.</p>
            <p>Women who perceived supportive community norms toward maternal healthcare were more likely to initiate ANC during the first trimester. This study result was agreed with a study conducted in southwest Ethiopia,
                <xref ref-type="bibr" rid="ref27">
                    <sup>27</sup>
                </xref> and western Ethiopia.
                <xref ref-type="bibr" rid="ref19">
                    <sup>19</sup>
                </xref> Community attitudes and social norms strongly influence women&#x2019;s healthcare-seeking behavior in many Ethiopian settings. Restrictive cultural practices, misconceptions about pregnancy disclosure, and gender norms may contribute to delayed care-seeking. Strengthening community engagement, involving local leaders, and expanding community-based maternal health education programs may improve timely ANC initiation.</p>
            <p>Overall, the findings of this study highlight that early ANC initiation is influenced by a combination of maternal knowledge, marital structure, community support, and health system responsiveness. Strengthening health education, reducing waiting times, and promoting supportive community environments are essential strategies to improve timely ANC initiation. Health facilities should reinforce counseling on the importance of early ANC initiation, while policymakers should prioritize community-based interventions and male partner involvement to improve maternal health service utilization. Furthermore, future research should explore deeper socio-cultural and health system factors influencing early ANC initiation, particularly in relation to marital dynamics and community-level barriers, to inform more targeted interventions.</p>
            <sec id="sec23">
                <title>Strength and limitations of the study</title>
                <p>A major strength of this study was its comprehensive assessment of factors associated with early antenatal care (ANC) initiation, incorporating socio-demographic, obstetric, and healthcare-related variables to provide a broader understanding of Early ANC initiation. However, institutional-based case-control study has several limitations. Firstly, although case-control studies are useful for identifying associations, temporal relationships between some exposures and early ANC initiation cannot be definitively established. Secondly, since it was conducted in a referral hospital, the findings may not be generalizable to all women in the community. In addition, recall and social desirability biases may affect the accuracy of self-reported data, particularly regarding missed doses and socioeconomic status while incomplete ANC registration records affected the accuracy of gestational age data.</p>
            </sec>
        </sec>
        <sec id="sec24" sec-type="conclusions">
            <title>Conclusions</title>
            <p>This study identified important determinants of early antenatal care initiation among pregnant women attending ANC services at Bule Hora University Teaching Hospital. Maternal age, knowledge of ANC, waiting time before consultation, marital structure, and community support were significantly associated with early ANC initiation. Therefore, to improve early initiation of antenatal care (ANC), strengthening Health Extension Program activities is essential, with particular emphasis on enhancing maternal health education, increasing community awareness and engagement, reducing waiting times at health facilities, and fostering supportive social environments. In addition, collaborative efforts among policymakers, healthcare providers, and community stakeholders are needed to strengthen maternal health services and address barriers that contribute to delayed ANC initiation. Furthermore, promoting male partner involvement programs is important to enhance timely ANC initiation.</p>
