Author notes that he is principal (owner) of DMWE DENTAL AND MEDICAL WRITING AND EDITING, Royersford, PA USA (a for-profit writing, editing and publication-planning business), is a member of American Medical Writers Association, International Society for Medical Publication Professionals, and Board of Editors in the Life Sciences. The paper was written by the author, without content analysis or editorial assistance from any of these or any other organizations referred to in the paper. Concurrent with the integration of appropriately cited content from literature sources here referenced, the paper represents the author's independent analysis, and is in no way attributable to any organizational entity referred to in the paper. Author has received no financial compensation from any entity or person in connection with the paper. In keeping with American Academy of Periodontology (AAP) registration procedures, author has, as a journalist, received complimentary media (press) registrations at the 2011 and 2012 Annual Meetings of AAP.
